What is SynapseRx?
SynapseRx is the consultation engine inside CommandRx, built for pharmacist-led medication reviews at independent community pharmacies. The pharmacist or a trained team member runs a structured intake, and SynapseRx produces the working documents of the review. The pharmacist reviews, edits, and owns every clinical decision. SynapseRx is a professional tool operated by pharmacy staff. Patients receive the handout and the counseling conversation, never the software.Structured Intake
Medications, supplements, symptoms, and labs, captured by voice, camera, or keyboard
Evidence-Checked Analysis
Interactions, depletions, timing, and risk flags, with citations checked against trusted sources
Script and Patient Handout
A consultation script the pharmacist reads, and a plain-language handout the patient keeps
SBAR to the Prescriber
A structured SBAR report delivered over HIPAA-compliant email, with delivery tracked
The workflow
- Intake. Enter the patient’s medications and supplements, chief complaints, symptoms, and optional details like height and weight. Import a medication list from a photo or screenshot, upload lab work, or run the intake by voice.
- Analysis. SynapseRx produces a clinical analysis: drug-drug and drug-supplement interactions, nutrient depletion signals, timing considerations, risk flags, and a patient-friendly summary. Citations are verified after generation; anything that fails the check is dropped rather than shown.
- Consultation. The pharmacist counsels from a generated script, edits freely, and prints or sends the plain-language handout home with the patient.
- Follow-through. Send the prescriber a structured SBAR in one click, and compile MTM documentation from the same review.
Why the review pays
Medicare’s CY2025 rule change roughly doubled the number of Part D patients plans must offer medication therapy management, and published benchmarks put reimbursement at 75 per completed comprehensive medication review. The constraint has always been pharmacist time. SynapseRx exists to make the review fast enough to be worth doing, without taking the pharmacist out of the loop. See the MTM expansion guide and run your own numbers with the MTM estimator.Getting started
Independent pharmacies enroll with their NCPDP number at app.rxvipnetwork.com/sign-up. SynapseRx is free during the pilot program.SynapseRx supports the pharmacist’s professional judgment. It is not a diagnostic device, and it does not make clinical decisions. Program reimbursement rates are set by plans and vary; verify against current program documentation.
